QuestionnairesEvery day we receive a large volume of email and telephone inquires regarding employment problems. It is impossible for us to schedule in-person appointments for everyone who calls our office. Over the years we have found the questionnaire to be an efficient device in helping us decide which employment-related cases we can handle. After your initial phone or email contact we may ask you to fill out one of the detailed questionnaires found in the sidebar on this page. Please do not fill in a questionnaire unless our office has specifically requested you to do so. If you are asked to complete a questionnaire, please print, complete, and mail the questionnaire to our office at the address below. If we believe we can help you, we will send you a letter asking you to schedule an in-person appointment. If we believe we cannot help you, we will try to refer you to other attorneys, government agencies, or community organizations that may be able to help. You may expect a response from our office approximately one week after we receive your completed questionnaire. Please download and print the questionnaire then fill it out as completely as possible.
